If you’re a long term reader here you don’t need any recommendation to Head & Shoulder and I use it myself, good product if you suffer dandruff, excma and psoriasis.

The offer isn’t dated, but restricted to 40,000 samples; it sounds a big number but Head & Shoulders samples are always popular. At a guess I’d get in this month (November 2012) and as quick as you can, but it might be available slightly longer.
